UM CMG Bolton Hill Practice Merges with UM CMG Heritage Crossing Primary Care

PRIMARY CARE PRACTICES MERGING IN BALTIMORE CITY
Two University of Maryland Community Medical Group (UM CMG) primary care practices in Baltimore City are merging, creating one 5,000 square-foot medical office with more than 30 clinicians providing care across multiple medical specialties.
Effective today, UM CMG’s existing Bolton Hill practice located on West Mount Royal Avenue has relocated to an expanded Heritage Crossing practice located at 312 N. Martin Luther King, Jr. Blvd., Suite 302. With the merger, the space for patients of both practices is doubled.
UM CMG – Primary Care at Heritage Crossing offers a range of health care services including checkups, flu vaccinations, care for minor illnesses and injuries, health screenings and ongoing management of health issues such as heart disease, diabetes and asthma. In addition to primary care services and care coordination, the Heritage Crossing practice includes clinicians specializing in neurology, psychiatry, psychology and rheumatology and on-site laboratory services. Heritage Crossing is a Patient Centered Medical Home practice, designated by the National Committee for Quality Assurance, as it focuses on patient-centered, coordinated care that provides accessible services to patients.

UM CMG is holding an Open House on November 17, 5 – 7 pm. at the Primary Care at Heritage Crossing practice. Community members can meet the practice’s doctors, tour the newly renovated office, participate in free health screenings and education and enjoy refreshments.
UM CMG currently consists of community-based provider practices from UM Baltimore Washington Medical Center, UM Charles Regional Medical Center, UMMC Midtown Campus and UM Shore Regional Health. A list of UM CMG providers is available at http://docfind.umms.org/cmg.
